When you have a "form" attached to your repeat prescription, your authorised medication is coming to it's end. The colour of the form depends on what is required in order for you to have your repeat medication re-authorised.
Forms Explained
Pink Form
Green Form
Blue Form
Used where blood tests are needed and then a review of your medication with a GP.
Used where blood tests are needed and review with a nurse.
Used where a patient has Asthma or Chronic Obstructive Airways Disease (COAD) ONLY without other illnesses and must be reviewed in our chest clinic.
Used where a patient is diabetic or has a heart condition and is not under the care of the hospital. Usually means blood tests and an appointment with the specialist nurse
